Safety assessment/ PAAG/ HAZOP
We at REACh ChemConsult GmbH give advise on and perform safety assessments using HAZOP. HAZOP (Hazard and Operability study)/ PAAG is a method for analysing the safety of planned or existing chemical plants. P A A G is divided into the following sub-steps:
- Prediction of malfunctions
- Identifying the causes
- Estimating the effects
- Evaluate countermeasures
Advice on potential hazards of process engineering systems
- Trapped energy (pressure, temperature)
- Exothermic energy, fire, explosion caused by chemical substances, quantity and properties of substances, fire spread, escape routes (explosion protection & fire protection issues)
- Energies, power-driven machines (machine protection issues)
- Ignition sources
- Exposure of personnel to substances, energies and moving parts, escape routes (occupational health and safety issues)
- Substance leakage (environmental protection issues)
- Operating errors (organisation)
- Access by unauthorised persons (unrecognised interference with process and system)
Advice on the safety concept of a plant/ system
- Inherent safety: e.g. substitution of hazardous substances, generation/immediate conversion, pumps with limited pressure generation < pressure design
- Primary safety measures (event-preventing, prevention): secure containment, i.e. the tightness of the outer enclosure under all circumstances
- Passive measures (e.g. pipelines, containers, closed non-return valves; always available without restriction (provided they are not pre-damaged)
- Design of the system against the impacts of normal operation
- Design of the system against the impacts of faulty operation
- Active measures (e.g. pumps, valves, transmitters, signalling devices) that prevent system conditions that could lead to a failure of the tight enclosure.
- Protective measures are designed in such a way that individual faults can be tolerated (single fault tolerance principle, 2-channel design)
- Operating measures: single-channel design

The new EU standard 61511-3 (2017) "Functional safety" describes the determination of tolerable risk as well as "probabilistic" methods for analysing and controlling risks in technical processes and systems in the process industry through appropriate protection systems and the specification of their necessary reliability.
LOPA is based on a risk matrix specified by the company and particularly critical individual scenarios from HAZOP studies.
